MINIMUM QUALIFICATIONS: Master s Degree, preferably in Social Work, Human Services or Counseling (copy required upon initial hire date),Direct experience working with youth and families in a counseling position (copy required upon initial hire date), Valid NYS Driver s License (copy required)Physical and TB Test (required within 1 month of initial hire date)
PREFERRED QUALIFICATIONS: Licensed Master s Degree in Social Work Previous experience in working with critically disturbed adolescents and families HOURS: Monday Friday 40 hours per weekMust be flexible to meet the needs of the families
POSITION OBJECTIVE: To effectively collaborate with families and or permanency resources in order to implement a comprehensive aftercare plan upon successful reintegration into the community.
DUTIES AND RESPONSIBILITIES:A. Locate, identify, and establish appropriate primary and alternative discharge resources for Raise the Age (RTA) residents, in conjunction with their Permanency Planning Goal, as defined by the Family Court System.B. Work closely with the RTA treatment team to stay informed regarding residents progress in treatment and effectively communicate progress to families.C. Provide family therapy and casework to the families and or permanency resource as indicated in the treatment plans. Keep families and resources updated about their youngsters progress in treatment. There should be bi-weekly face-to-face contact with families whose youngster is currently in placement. D. Provide liaison services to the referral agencies and other human services agencies in the community in order to facilitate the youngster s positive reunification back into their home community. E. Provide after-care services to the RTA youngsters and their families for four (4) months after discharge from the Agency. F. Be an integral part of the child s treatment team by providing input to the team about family issues, dynamics, and needs by participating actively in the formulation of treatment plans.G. The Aftercare Services Coordinator will meet on a regular basis with the Residential Department Head for supervision. H. If needed, represent the Agency at Family Court hearings and present the Agency s assessment and recommendation regarding the child and family s treatment needs.I. Help develop discharge plans. Make referrals for aftercare services, including but not limited to services through Office for People with Developmental Disabilities, Bridges to Health, Mental Health, etc. Assist families in identifying and securing aftercare resources.J. Identify and establish educational, vocational, and aftercare resources in local communities for the RTA youth upon discharge. K. Perform other duties required.L. Responsible for adhering to and performing all duties in accordance with safety standard, practice and policy of The William George Agency.M. Adhere to all rules, regulations, policies and philosophy of The William George Agency and the New York State Office of Children and Family Services.
